PRN Home Health Physical Therapist: Looking for passionate therapist to
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on evidence based care and patient centered approach
- Provide skilled therapeutic interventions to improve strength, balance, mobility, coordination, endurance, and functional independence.
- Assess patients for fall risk, safety concerns, functional limitations, and the need for appropriate assistive devices or home modifications.
- Provide gait training, transfer training, therapeutic exercise, balance activities, and functional mobility training.
- Educate patients and caregivers on home exercise programs, safety precautions, fall prevention, energy conservation, and strategies to maximize independence.
- Monitor patient progress and modify treatment plans as clinically appropriate to achieve established goals.
- Complete timely, accurate, and compliant documentation, including evaluations, daily treatment notes, reassessments, progress reports, and discharge summaries.
- Communicate effectively with other members of the interdisciplinary team.
- Identify changes in patient condition and communicate significant clinical findings or concerns to the appropriate healthcare providers.
- Participate in care coordination and discharge planning to promote safe transitions and continuity of care.
- Maintain compliance with state licensure requirements, Medicare guidelines, company policies, and applicable home health regulations.
- Maintain appropriate professional boundaries and represent the therapy company with integrity and professionalism.
- Manage a flexible PRN caseload while maintaining timely communication and documentation standards while maintaining high quality patient care.

Company Description
KRS is a Comprehensive Outpatient Rehabilitation Facility that functions primarily as a staffing agency for home health agencies. We primarily work with selected Home Health Partners in Pinellas and Pasco Counties who provide Patient Driven Care. We have a few ALFs that we are preferred providers in as well. We are blessed to have outstanding clinicians who work together to keep our patients aging in place and safe in their homes. Many of these clinicians have been with us for over 15 years. Over the last 20 years we continue to grow despite an every changing healtcare system. Our goal is to continue to put our patient and employees first.
