Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Arizona state PT license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience in an acute hospital setting preferred based on job description duties.
- Ability to evaluate patients, develop and modify treatment plans, and document accurately in medical records.
- Demonstrated competency in safe patient handling and materials handling procedures.
- Maintain professional licensure and complete state‑mandated continuing education.
Preferred Qualifications
- Prior experience working with interdisciplinary teams in a hospital environment.
- Experience with discharge planning, equipment trialing, and patient/family education.
- Participation in student supervision or clinical education preferred.
Assignment Details
- Part-time schedule: 2 shifts per week, 8 hours each.
- Approximately 8–24 hours per week.
- May be required to provide coverage as needed and collaborate with interdisciplinary team.

Impacting Patient Care
Evaluate assigned patients and develop treatment plans that progress patients toward discharge within an established length of stay. Collaborate with interdisciplinary team members to address barriers to discharge, complete equipment trialing and ordering, and provide patient and family training including home exercise programs. Complete timely, accurate documentation including daily notes, goal writing, letters of medical necessity, and discharge documentation. Participate in professional development and performance improvement initiatives. Uphold professional codes of conduct and organizational safety expectations.
