Position Summary

We are seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing team. The ideal candidate will have experience in hospital medicine, skilled nursing, long-term care, rehabilitation, or a related setting. This role requires strong clinical judgment, excellent communication skills, and the ability to work collaboratively with patients, families, physicians, and facility staff.

Qualifications

Active Nurse Practitioner or Physician Assistant license required
Experience in hospital, skilled nursing, long-term care, or rehabilitation settings preferred
Knowledge of EMR systems preferred
Strong computer skills, including the ability to manage patient lists, problem lists, email, and documentation systems
Ability to complete timely and accurate documentation and coding
Excellent communication skills, including professional phone communication with families, physicians, and other healthcare professionals
Strong organizational skills and attention to detail
Team-oriented mindset with a positive and professional attitude
