The Licensed Practical Nurse will provide attentive and compassionate care to a mixed resident population at The Madison Health and Rehab during the 3 p.m.–11 p.m. Monday–Friday shift. Reporting to the Director of Nursing and working with a dedicated evening-shift team, the LPN will support residents’ clinical needs while maintaining accurate documentation and effective communication with families.
$25.50 per hour + $2 shift differential
Responsibilities
- Provide patient care to a mixed resident population.
- Administer medications safely and accurately.
- Monitor and document vital signs.
- Provide wound care according to established care plans.
- Complete accurate clinical documentation.
- Monitor patients and report changes in condition.
- Communicate effectively with residents’ families.
Preferred Qualifications
- Active Licensed Practical Nurse (LPN) license.
- Basic Life Support (BLS) certification.
- Experience with medication administration and vital signs monitoring.
- Wound care experience.
- Proficiency with electronic health records and clinical documentation.
- Experience with patient assessment.
- Compassionate communication skills.
