Job Summary
We are seeking a Licensed Practical Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a LPN who operates with great attention to detail and a big heart. As a LPN with Gifted Handz LLC we care for those with Intellectual Disabilities in their homes and require a nurse who can collaborate, work independently, good people skills and nursing judgement and who is reliable. You will provide 1:1 care and support to your individual. I have several clients needing assistance in Straustown Pa, and Philadelphia Area
Responsibilities
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications
- Currently licensed in state of employment
- Previous experience as a LPN is preferred however new grands are welcome
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification required
