If you’ve built your career managing complex chronic conditions — titrating medications, watching labs trend in the right direction, knowing a patient’s whole story — this is the practice you’ve been looking for.
Chandra Britt Armstrong MD, LLC is an independent family medicine practice in Decatur, Georgia, and we are searching for a Family Nurse Practitioner who is clinically confident, relationship-driven, and ready to manage a panel of patients who genuinely need you.
This is not an urgent care position. This is primary care medicine, the real thing.
ABOUT THE POSITION
We are offering a part-time position — Thursday and Friday — with a clear path to full-time as your patient panel grows. We expect this schedule to expand to five days within 12 to 18 months based on patient demand.
You will practice primary and preventive care under the supervision of Dr. Chandra Armstrong, a board-certified family physician who has served this community for over twenty years.
What you will actually be doing:
Managing patients with Type 2 diabetes — tracking A1c trends, titrating medications, educating patients through real conversations about real change.
Managing hypertension, often alongside CKD, CHF, or COPD — adjusting regimens, watching kidney function, making clinical decisions that matter over months and years.
Caring for an older adult population, predominantly African American, many of whom have trusted this practice for decades. These are long-term relationships. Your clinical decisions will compound over time.
Conducting preventive care visits, annual wellness exams, and transitional care follow-ups.
Collaborating directly with Dr. Armstrong on complex cases and referring intelligently when specialist involvement is warranted.
WHO WILL SUCCEED IN THIS ROLE
You have at least two years of experience managing chronic conditions in a primary care, internal medicine, or family medicine outpatient setting.
You are comfortable making longitudinal clinical decisions — not just responding to today’s chief complaint, but tracking where a patient has been and planning where they need to go.
You understand that a well-controlled A1c represents months of relationship, not a single prescription.
You communicate clearly with patients who may have limited health literacy, who may have had bad experiences with healthcare systems in the past, and who require trust before they accept guidance.
You see twenty patients in a day and leave clinical notes that reflect actual thinking — not cut-and-paste templates.
You adapt quickly, you ask good questions, and you are comfortable being held accountable for patient outcomes.
THIS POSITION IS NOT FOR YOU IF:
You prefer episodic care and are not interested in longitudinal patient relationships.
You are not comfortable managing multi-morbid elderly patients without a specialist in the room.
You want a stable, low-expectation environment where productivity is not tracked.
You are uncomfortable with technology or resistant to AI-assisted documentation.
(We are explicit about this because your time and ours are both valuable. The right candidate will read this and feel relieved, not warned off.)
WHY INDEPENDENT MEDICINE
We are not a health system. We are not a corporate clinic. We are not owned by a private equity group.
Dr. Armstrong makes the clinical and operational decisions for this practice. That means when you have an idea, you can bring it directly to the physician who leads this organization. When a patient needs something creative, you have the latitude to provide it. When you want to develop a new skill, the conversation starts tomorrow — not in the next annual review cycle.
We participate in the Emory Healthcare Network, which gives our patients access to strong specialist relationships without the bureaucracy of practicing inside a large system.
OUR PATIENTS
Our patient panel is primarily African American adults and seniors in the Decatur, Georgia community. Many are 60 and older. Many have been patients of this practice for more than a decade.
They bring complexity. They bring history. They bring trust that was earned over years.
The right provider sees that as the best part of the job.
TECHNOLOGY AND SUPPORT
We use AthenaOne EMR with CarePilot AI medical scribing, which means you spend your time with patients, not with documentation.
We have on-site laboratory capability, a dedicated medical assistant, and administrative support. We have invested in removing unnecessary friction from your clinical day because we believe your time is best spent on patient care.
We are also building expanded service lines — including IV wellness infusions, pelvic floor rehabilitation with BTL Emsella, body composition therapy, and medical aesthetics — and we will provide training for providers who are interested in expanding their clinical skills into these areas over time.
COMPENSATION AND GROWTH INCENTIVE
Hourly Rate: $82 – $95 per hour, commensurate with experience
Beyond base compensation, eligible providers participate in a clinical growth incentive program tied to patient panel growth, patient satisfaction outcomes, and operational excellence. We believe the providers who build this practice should share in what they build.
As your panel grows and the schedule expands to full-time, compensation structures will be revisited accordingly.
