* Build and maintain positive, professional relationships with clients experiencing a range of mental health conditions.
* Conduct comprehensive needs assessments to identify client strengths, challenges, and service requirements.
* Develop individualized treatment plans in collaboration with clients to guide services and overall case direction.
* Conduct community-based visits with clients at least twice per month to monitor progress and provide ongoing support.
* Assist clients with accessing community resources, including referrals for therapy, employment services, transportation, housing assistance, food pantries, and medical healthcare clinics.
* Teach and reinforce coping skills and life skills to promote independence and emotional well-being.
* Provide support to clients who have experienced a mental health crisis, ensuring appropriate follow-up and coordination of care.
* Document all client visits and case activities within 24 hours of service delivery in accordance with agency policies.
* Communicate and collaborate with community resources (e.g., homeless shelters, healthcare providers, and social service agencies) to ensure continuity of care.
* Attend and actively participate in Treatment Team Meetings with the Clinical Director, physicians, clinical team, residential team, and other staff to coordinate services and promote positive client outcomes.
* Collaborate effectively with internal teams and external community stakeholders.
The Client Service Worker provides direct care, supervision, and support to individuals with intellectual and developmental disabilities at a day program. Client Service Workers assist individuals in achieving personal goals, developing independent living skills, participating in community activities, and maintaining their health and safety while promoting dignity, respect, and person-centered services.
