Position Summary
The LPN supports primary care services by providing nursing care within the scope of their license, assisting providers, and coordinating patient follow-up. This position works closely with clinical and administrative staff to help patients receive timely, organized care.
Responsibilities
- Prepare patients for visits, obtain vital signs, and document medications, allergies, symptoms, and relevant health information.
- Provide nursing care, administer medications and vaccines, and perform ordered treatments within authorized scope and clinic policy.
- Assist providers with examinations, procedures, and clinical services.
- Perform specimen collection and approved laboratory testing according to training and clinic procedures.
- Communicate patient concerns and abnormal findings promptly to the appropriate provider.
- Support patient education and reinforce provider instructions on medications, preventive care, and chronic conditions.
- Respond to patient calls and messages within assigned responsibilities and escalate concerns requiring provider assessment.
- Assist with referrals, test-result follow-up, hospital discharge follow-up, and care coordination.
- Support care-gap outreach, preventive screenings, and quality improvement activities.
- Maintain accurate, timely documentation in the electronic health record.
- Follow infection prevention practices and maintain clean, stocked clinical areas.
- Monitor supplies, medication storage, and vaccine temperatures as assigned.
- Protect patient privacy and follow clinic policies and safety procedures.
- Perform other duties reasonably related to the position.
Required Qualifications
- Current, active Florida LPN license.
- Current BLS/CPR certification.
- Strong communication, organization, and documentation skills.
- Ability to work effectively with patients and colleagues from diverse backgrounds.
- Comfort using an electronic health record and basic computer applications.
- Ability to manage competing priorities while maintaining safe, attentive patient care.
Preferred Qualifications
- Experience in primary care, an outpatient clinic, or a community health center.
- Experience with immunizations, specimen collection, and chronic disease care.
- Familiarity with patient outreach, care coordination, and quality improvement.
- Bilingual English/Spanish skills.
Work Environment
This is an on-site position in Ocala, Florida. Duties involve direct patient contact, standing and walking, and handling clinical supplies and equipment. Essential functions may be performed with reasonable accommodation.
