Home Health LPN – 13-Week Travel Contract
KPG Healthcare is seeking an experienced Home Health LPN for a full-time 13-week travel assignment. The LPN will provide skilled nursing care to patients in their homes, completing scheduled visits and managing care based on each patient's individual needs.
Responsibilities:
- Provide skilled nursing care to patients in their homes
- Complete scheduled in-home patient visits and provide care based on the plan of care
- Monitor patient condition and report changes to the RN or supervising provider
- Administer medications and treatments as ordered
- Perform wound care and other skilled nursing procedures as needed
- Reinforce patient and family education and care instructions
- Communicate with the RN, physicians, and other members of the healthcare team
- Document visits using Homecare Homebase (HCHB)
Requirements:
- Active LPN license in the state of practice
- 1+ year of recent Home Health experience
- BLS certification (AHA)
- Homecare Homebase (HCHB) experience preferred
