The LPN at The Madison Health and Rehab plays a critical role in providing comprehensive nursing care to patients, primarily managing a caseload of up to 15 patients. This position operates within both nurse-only teams and multidisciplinary healthcare teams, ensuring high-quality care through close coordination and communication.

Responsibilities
- Provide direct patient care including basic nursing services
- Administer medications safely and accurately
- Monitor and record vital signs regularly
- Perform wound care following established protocols
- Maintain detailed and accurate patient documentation
