We are seeking a Licensed Practical Nurse (LPN) with Home Health experience to provide both office-based nursing care and independent home visits within the local area. This role involves a balanced mix of office duties and hands-on patient care in their homes, supporting patient health through medication administration, vital signs monitoring, and patient education.
Responsibilities
- Conduct patient visits independently in the local area
- Provide nursing care and support within the office setting
- Administer medications and monitor vital signs during visits
- Maintain accurate and detailed patient records
- Educate patients on health management and care plans
- Communicate effectively with patients and healthcare team members
- Utilize standard medical supplies during home health visits
Required Qualifications
- Licensed Practical Nurse (LPN) with a valid LPN license
- Minimum of 1 year experience in licensed practical nursing
- Experience in Home Health Care
