We are hiring an Occupational Therapist (OT) for a Home Health position providing skilled care to patients in their homes in the Mount Prospect area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:
- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
- Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
- Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
- Work independently, but never alone
- Travel to patient homes within assigned territory

OT Qualifications
- Active Occupational Therapist, OT license in the state you work
- One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
