WCS Healthcare Partners is seeking an RN Care Delivery Manager to join a large social services agency in New York City. This role is responsible for overseeing care delivery, coordinating interdisciplinary services, supervising staff, and ensuring high-quality, compliant care for a frail and elderly population.
Job Responsibilities:
- Oversee the development and execution of individualized care plans to improve participant outcomes and satisfaction.
- Coordinate and facilitate Interdisciplinary Team (IDT) meetings, Service Delivery Requests (SDRs), and other care coordination activities.
- Serve as the on-call manager during non-operational hours, addressing participant and caregiver concerns, grievances, and follow-up documentation.
- Monitor clinical operations, staffing coverage, and resource allocation to ensure seamless day-to-day site operations.
- Collaborate with interdisciplinary teams, clinicians, discharge planners, and community providers to ensure continuity of care.
- Review UAS assessments, participant charts, and clinical documentation for accuracy, quality, and regulatory compliance.
- Investigate incidents and ensure timely completion of Level 2 investigations and required documentation.
- Coordinate long-term care placements and transitions of care between hospitals, rehabilitation facilities, nursing homes, and home care settings.
- Supervise, coach, onboard, and evaluate staff while ensuring compliance with organizational policies and applicable state and federal regulations.
- Support quality improvement initiatives, maintain knowledge of evidence-based practices, and contribute to policy and program enhancements.
Minimum Qualifications:
- Active New York State RN License required.
- BSN required, or a minimum of five (5) years of experience in a healthcare setting.
- Minimum of two (2) years of administrative or management experience within a Certified Home Health Agency (CHHA), Licensed Home Care Services Agency (LHCSA), acute care, medical-surgical, critical care, nursing home, or diagnostic and treatment clinic setting.
- Minimum of one (1) year of experience working with a frail or elderly population.
- Supervisory experience required.
- Completion of NYSED-approved infection control coursework required.
- A minimum of 8.0 education contact hours, which may include mandatory training requirements such as infection control coursework.
- Bilingual skills preferred.
