We are seeking a collaborative Nurse Practitioner or Physician Assistant to provide exceptional care in a patient-centered environment. This role offers the opportunity to build your own patient panel with ample support for seamless integration into our team.
Responsibilities:
- Provide high-quality primary care services to adult patients (ages 18 and up) in an outpatient setting.
- Develop and manage personalized care plans for patients, ensuring comprehensive and continuous care.
- Collaborate with a multidisciplinary team to enhance patient outcomes and ensure a cohesive approach to healthcare delivery.
- Maintain accurate and timely documentation of patient encounters and treatment plans.
- Participate in quality improvement initiatives to enhance patient care and practice operations.
Requirements:
- Licensure: Unrestricted State of Connecticut Medical License (or eligible)
- Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
- Experience: 1 year of experience in a primary care or relevant outpatient setting
Setting: 100% Outpatient Primary Care – Adult Medicine (18+)
