Join a home health team in Cedartown, GA as a traveling Physical Therapist where you will evaluate and treat patients in their homes, help patients recover from injuries, and manage chronic conditions while experiencing work in a new location. You will complete comprehensive evaluations, establish measurable goals, develop and revise treatment plans, communicate with the interdisciplinary team and physicians, document according to company and regulatory requirements, and supervise PTAs and aides as permitted by state law.
Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
- Active Georgia Physical Therapist license required to start the assignment.
- BLS certification from American Heart Association or American Red Cross may be required.
- Experience with home health settings preferred.
- Experience with HCHB (Home Care HomeBase) and OASIS required.
- Productivity experience or ability to meet productivity expectations (30 points) preferred.
- Ability to perform charting, billing documentation, and maintain compliance with state and federal regulations.
- Mileage reimbursement policy noted; candidate must be able to travel to patient homes across service counties.
Preferred Qualifications
- Previous travel or contract physical therapy experience preferred.
- Experience supervising PTAs and aides preferred.
- Prior experience working with Bartow and Paulding County patient populations preferred.
Assignment Details
- Shift Requirements: Monday through Friday schedule as specified by client.
- Typical weekly schedule: five 8-hour shifts.
- Counties covered: Bartow and Paulding Counties.
- Onsite requirement in patient homes. Candidate must travel locally to patient residences.
