Join a home health team in Yuma, AZ as a traveling Physical Therapist. Provide patient evaluations and therapeutic services according to physician orders. Supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned. Perform home visits, evaluate the home environment, and participate in interdisciplinary care and in-service education. Maintain accurate documentation and communicate patient status to the interdisciplinary team.
Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or an equivalent accrediting body.
- Current, active, and unencumbered license to practice physical therapy in the state of assignment.
- Familiarity with home health practice requirements, including conducting home visits and evaluating home environments.
- Current CPR certification.
- Reliable transportation and a valid driver’s license.
- Ability to use basic computer functions for documentation, email, and online training.
- Client requirement: experience providing or supervising physical therapy services in home health settings.
Preferred Qualifications
- 2–3 years of physical therapy experience preferred.
- Experience supervising PTAs and coordinating care in home health agencies preferred.
Assignment Details
- Shift: Evenings, 5x8.
- Must be able to perform home visits and travel within the assigned service area.
