Physical Therapist — Outpatient | Surprise, AZ
Provide outpatient physical therapy services focused on orthopedic patients. Evaluate assigned patients and develop, implement and modify treatment plans according to patient needs, goals and physician direction. Delegate and direct discipline-specific therapy services for assigned patients and design functional treatment programs to support return to community activities. Educate patients, families, students and coworkers. Complete documentation, goal writing, functional quality indicator assessment, letters of medical necessity and charge entry in a timely and compliant manner. Demonstrate compliance with professional standards, organizational policies and safety practices.
Minimum Requirements
- Bachelor’s, Master’s or Doctoral degree in Physical Therapy from an accredited program approved by APTA.
- Active Arizona state license in Physical Therapy required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience in outpatient orthopedic settings preferred.
- Ability to complete timely, accurate documentation and use electronic medical record systems.
- Demonstrated competence in safe patient handling and materials handling procedures.
Preferred Qualifications
- Previous travel or contract therapy experience preferred.
- Experience supervising or precepting students preferred.
- Additional certifications related to orthopedics or manual therapy preferred.
