At Duke Home Health, you are able to meet patients where they are. By working as a home health nurse, you help keep patients out of the hospital and in their homes where they are most comfortable. Duke HomeCare and Hospice provides innovative, team-based care focused on achieving the best outcomes for patients, families, and the communities served. In this role, you will connect patients to the right level of care at the right time.
Schedule:
- Monday-Friday – 8 hour days

How you will make an impact
- Deliver and document professional nursing care in home settings using the nursing process in accordance with North Carolina regulations and agency standards.
- Conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams to deliver effective care.
- Educate patients and families on disease processes, medications, and care plans to support outcomes and reduce hospital readmissions.
- Coordinate care including case management responsibilities, discharge planning, and OASIS assessments tied to reimbursement and quality outcomes.
- Ensure compliance with regulatory, safety, and quality standards while maintaining accurate and timely documentation.
What you will need
- Associate Degree or Diploma in Nursing required; BSN encouraged after two years of service.
- Current RN license in North Carolina or compact state.
- CPR certification (American Heart Association).
- Valid driver’s license and current automobile insurance coverage.
- 1–2 years of med-surgical, critical care, or home health experience preferred.
- Knowledge of Medicare and Medicaid regulations, nursing scope of practice, and infection control standards.
- Strong assessment, critical thinking, communication, and documentation skills with the ability to work independently in home settings.
