Navigator RN - Heart and Vascular at Providence Health and Services summary:
The Cardiac Nurse Navigator coordinates care for CABG and other cardiac surgery patients across pre-procedure, inpatient, discharge planning and post-hospital follow-up, applying cardiac evidence-based practice and collaborating with clinicians to close care gaps. Requires a BSN, Washington RN license, BLS, and 3+ years (preferred 5) cardiac surgery RN experience. Role is full-time, on-site at Swedish Cherry Hill within Providence, focused on patient-centered continuity of cardiac care and clinical coordination.
Description
The cardiac nurse navigator serves as a consistent resource to support optimal care delivery for coronary artery bypass graft (CABG) patients and other projects as assigned, focusing on coordination of care during pre-procedure, hospitalization, discharge planning and facilitating post-hospitalization follow-up. The nurse navigator is knowledgeable of multiple treatment decisions, cardiac evidence-based standards, best practice guidelines and maintains care collaboratively across the continuum. In collaboration with clinicians, identifies and facilitates optimum cardiac care delivery with clinical expertise, strong interpersonal abilities and critical thinking skills. Provides effective oral or written feedback to providers to identify care gaps.

Required Qualifications:
- Bachelor's Degree in Nursing from an accredited school, with a focus on cardiac surgery inpatient care.
- Washington Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 3 years of experience as a Registered Nurse in cardiac surgery.
Preferred Qualifications:
- 5 years of experience as a Registered Nurse in cardiac surgery.
