Job Summary This position will work directly with patients on disease specific education, medication reconciliation, education, creates individualized care plans and interventions and attends physician’s appointments with patients as needed. Collaborates with medical providers, primary care teams, and social services agencies to provide care coordination and connection to resources and support to program clients to improve their health and general well-being through education and provision of coordination of care and services. Works in both clinical and community-based settings, including client's homes. Qualifications Education Required High School Diploma, GED, or equivalent Completion of an accredited program of nursing Associate's degree in nursing Preferred Bachelor's degree in nursing Master's degree in nursing Experience: Required Two or more years of nursing experience Certification/Licensure: Required Registered Nurse (RN), with Authorization to Practice in the State of Texas Basic Life Support (BLS) certification required with ninety (90) days of start date Valid Texas Driver's License
