Summary:
We are seeking a dedicated Physician to join our team in building and managing a growing panel of patients within independent and assisted living facilities. In this role, you will have the unique opportunity to establish and nurture relationships with residents while overseeing their long-term and preventive care needs. You will work collaboratively with Curana Health’s care teams and facility staff to deliver exceptional, consistent care in the convenience of the patients' residences.
Essential Duties & Responsibilities:
- Patient Panel Development: Build and maintain a dedicated patient panel by establishing relationships with new residents and managing ongoing care for established patients within assigned facilities.
- Direct Patient Care: Conduct comprehensive assessments, manage chronic conditions, and provide preventive care services for residents, ensuring continuity of care as their primary physician.
- Routine Rounds: Perform regular rounds at assigned facilities, addressing both chronic and acute health concerns.
- Care Coordination: Collaborate with facility staff, family members, and interdisciplinary teams to create and manage personalized care plans tailored to each resident's needs.
- Documentation and Compliance: Ensure accurate, thorough documentation in the EMR system, adhering to regulatory requirements and Curana Health’s policies.
- Patient Education: Educate residents and their families on health management, preventive care, and available resources.
- Quality Assurance: Participate in quality improvement initiatives, analyzing performance metrics to continually enhance patient outcomes.

Qualifications:
- Medical Degree (MD or DO) from an accredited institution
- Board Certification in Family Medicine, Internal Medicine, Geriatrics, or Hospital Medicine
- Active, Unrestricted Medical License in the state of practice
- DEA Certification and current BLS/ACLS certification
- Proficiency in EMR systems and timely documentation practices
- Strong Interpersonal and Communication Skills, with a commitment to patient-centered care
This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required. If so, you will be provided with thirty (30) days notice of this requirement.
