Reporting to the Coordinator of Care you will be responsible for seeing patients in their home for revisits based on a skilled need, implement the plan of care and evaluate the treatment. - NY State LPN License required
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility required
- Must have a NY State Drivers License and a car
