Support medication coordination and management activities appropriate to your license and scope of practice.

RESPONSIBILITIES (CONTINUED)

Collaborate with families, caregivers, healthcare providers, and community services.
Take initiative to identify concerns, address barriers to care, and complete follow-up.
Assist with coordination during unexpected hospitalizations, hospital discharges, and other care transitions.
Complete accurate, timely documentation in our electronic medical record system.
Maintain client confidentiality and follow applicable HIPAA requirements and company privacy and security policies.
Recognize situations requiring clinical guidance, provider involvement, or emergency services and escalate appropriately.

REQUIRED QUALIFICATIONS

Current, active California RN or LVN license in good standing.
Minimum of five years of clinical nursing experience as a licensed RN or LVN in an inpatient hospital, hospital-based outpatient clinic, ambulatory care, case management, care management, or a comparable clinical setting.

Strong communication, organization, and problem-solving skills.
Compassionate, professional approach to working with older adults and their families.
Ability to manage field responsibilities independently while working within your scope of practice and seeking guidance when needed.
Sound judgment, initiative, and dependable follow-through.
Flexibility to adapt to changing priorities and time-sensitive client needs.
Comfort using electronic medical records and secure digital communication tools.
Reliable transportation, a valid driver's license, and current auto insurance.

Experience supporting older adults and individuals with complex medical needs is strongly preferred.

SCHEDULING & AVAILABILITY

This role offers flexible hours and requires a high degree of flexibility in return. We are seeking someone with dependable availability who is willing to adjust their schedule to support time-sensitive client needs, such as an unexpected hospitalization, urgent appointment, or hospital discharge.

Prompt communication and reliable follow-through are essential. Scheduling expectations, response times, and any arrangements for urgent coverage or availability outside scheduled hours will be discussed and agreed upon in advance.
